On Admission

Please fill out the forms completely and legibly.
Present authorization letters and letter of guarantee, ID cards and Medicare/PHIC forms if you are a company patient.
Present a note, ID & membership cards from your Insurance Company if you are an HMO patient.
Medicare/PHIC members: Submit your completed Medicare/PHIC forms to the Medicare/PHIC Section (ground floor).

Payments

Cash payments are required on admission to cover medicines, supplies, diagnostic procedures and surgery
Periodic payments are required to maintain a hospital balance not to exceed P5,000.00
Updated hospital bill may be requested from the Nurse's Station.
Business Office Hours 6:00am-7:00pm Mondays to Saturdays 8:00am-5:00pm Sundays and Holidays
Credit Card Payments are accepted.
Check payments are accepted but subject to prior validation
